Nurse Practitioner, In-Home Assessments

Must be available to work a minimum of 10 hours per week - YOU create your own schedule!

LHI is looking for qualified and experienced Nurse Practitioners to join our network through an existing subcontractor to conduct in-home health assessments on enrolled Medicare Advantage members. A Nurse Practitioner will complete visits on members within a geographic area of responsibility.

Counties of need include:
Davidson, Buncombe, Haywood, Cabarrus, Rowan, Cherokee, Graham, Clay, Gaston, Lincoln, Granville, Vance, Warren, Greene, Wayne, Halifax, Northampton, Lee, Moore, Onslow, Pender, Person, Caswell, Rockingham, Stokes, Yadkin, Davie, Forsyth, Rutherford, Polk, Transylvania, Henderson, Jackson, Swain, and Macon.

General Responsibilities
o Conduct in-home assessments on health plan members. Average visit is 45-60 minutes in length.
o Assessment includes:
Past medical history, review of systems, physical examination, medication review, and depression screening.
o Responsible for checking vitals, conducting a physical exam that includes monofilament test, urine dipstick, and foot exam (as appropriate).
o Identify diagnoses to be used in care management and active medical management in the furtherance of treatment.
o Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.
o Communicate findings in your assessment that will be used to inform members PCP of potential gaps in care.
o Identify needs and opportunities to generate clinical referrals.
o Educate members on topics such as disease process, medication, and compliance.
o Compliance with all HIPAA regulations and maintain security of protected health information (PHI).

Required Education and Experience
o NP (license and national certification, or the ability to obtain national certification and/or NP license in state of assignment) active, unencumber licensure free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
o Minimum of 1 year clinical experience in their highest level of education, clinical setting preferred.
o Family, Geriatrics, or Adult certification required
o Experience in gerontology, cardiology, internal medicine, or endocrinology a plus.
o Home care or home visit experience a plus.

Required Knowledge, Skills, and Abilities
o Ability to obtain reliable transportation in order to complete home visit assessments
o Knowledgeable in regards to standard of care of common chronic health conditions as well as health promotion and prevention guidelines.
o Excellent administrative, organizational and verbal skills are required.
o Professional image and behavior are required.
o Strong communication skills with the geriatric or Medicare population.
o Ability to navigate on the internet and work with personal computer.
o Ability to work independently.
o Detail oriented.
o Dependable and reliable.
o Ability to lift 30 pound bag in and out of car and up and down steps.
o Ability to navigate stairs and a variety of dwelling conditions and configurations.
o Ability to sit, stand and kneel as needed to perform physical assessment.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
